New Patient Registration

Family history 0-1 year oldWe welcome new patients to the RM Pediatrics family and look forward to providing care to newborns and those new to the neighborhood.

RM Pediatrics accepts new patients through the age of 18, up to their 19th birthday.

Our Pediatrician treat our patients from birth through age 22. 

To set up your first appointment, call 408-297-9949 and speak to one of our scheduling representatives.

Due to the HIPAA privacy act, only you can officially transfer your records to RM Pediatrics. Please use our Release of Medical Information Form. Once completed, please send it to your previous clinic and they'll transfer your records to us. A Release of Medical Information Form needs to be filled out for each patient.

Once you've had your first visit with us, you will have the opportunity to utilize our Online Services which includes Online Appointment Scheduling, Bill Payment and Patient Messaging.

At your first visit please bring the following:

• A valid identification card
• Your health insurance card
